EMERGENCY CARE
One of the physicians is available for emergencies at all times. During office hours please call our office. Our
phone is answered 24 hours a day. After office hours, your message will be forwarded to the doctor on call. He
will return your call as soon as possible.
OFFICE HOURS AND APPOINTMENTS
The office is open Monday through Friday except holidays. Patients are seen by appointment. When you call for an
appointment you will be seen as quickly as possible. Every effort will be made to schedule a convenient appointment
with the physician of your choice. Each of the physicians has specific hours he is in the office, our scheduling
office will explain these to you. As your time is important to us, every effort is made to see patients on time
as scheduled. Emergencies do arise, they must take precedence and some delay may occur |

PRESCRIPTIONS
For routine requests for medicine refills please call your pharmacy. Your pharmacist will call the office to obtain
authorization for your refill. It is our policy not to fill narcotic prescriptions after hours, on weekends
or holidays. Please anticipate your prescription refill requirements so we can best serve your needs.
FEES AND INSURANCE
Your Insurance co-payment is due at the time of your appointment.
We accept many insurance plans. If your insurance requires authorization for an office visit, please contact your
referring physician for that authorization prior to calling for an appointment.
The responsibility for payment of fees for services rendered is the direct obligation of the patient.
Our physicians are participating Medicare physicians and do accept assignment on Medicare patients, but any deductible
or co-payment is the patient's responsibility.
Our billing office will provide assistance if needed with your insurance. We file claims for primary insurance
plans. Claims for Secondary insurance are filed for managed care plans only.
